Child Death Overview Process

We have a responsibility to look into the circumstances when a child dies, as set out in "Working Together to Safeguard Children."

We must arrange for:

  • A rapid response by a group of key professionals who come together for the purpose of enquiring into and evaluating each unexpected death of a child,
  • An overview of all child deaths in the LSCB area.

Reporting and Managing a Child Death

To notify us of the death of a child resident in Kent please follow the link below to complete your Form A within 24hrs.

Professionals required to complete a Form B will be contacted directly by KSCB of the NHS Child Death Review Service.
